Building a Medical Home Partnership: A Wisconsin Toolkit

All of the Wisconsin Medical Home Team members that you will meet in this Toolkit, are parents and providers who have demonstrated their dedication to children and youth with special health care needs and to the work of improving systems of care. We want to take this opportunity to thank them for taking time to share their expertise and allow our cameras into their offices and homes.  We are especially grateful to the families – parents and young people - who were so generous with their time and sharing their stories to make this Toolkit possible. 
                   
This Toolkit would also not be possible without the inherited wisdom and generously shared resources available on the American Academy of Pediatrics’ National Center of Medical Home Initiatives for Children with Special Health Care Needs Web site. 

In addition, we would like to thank the Wisconsin Chapter –American Academy of Pediatrics, Wisconsin Academy of Family Physicians and Family Voices of Wisconsin for their continued efforts to promote the concepts of Medical Home and this Toolkit among their members.
